I observed and helped my nurse use these five steps throughout the day. An example of when we used it was when one of our patients was in pain. The nurse assessed the patient's pain and asked for his subjective opinion on how he would rate his pain. Then the nurse made a nursing diagnosis of acute pain and created a plan to treat his pain. Finally, he implemented the plan by administering painkillers, repositioning the patient, turning on the TV for distraction, and dimming the lights. The patient was involved in the planning and implementation process through the nurse asking his opinion on his pain, what amount of pain would be tolerable for him and how he thought it would be best to relieve his pain.
