Parkinson's diseaseThe first scientist who discovered Parkinson's disease (PD) was an English doctor, James Parkinson. In 1847, Dr. Parkinson published an article entitled "Essay on Shaky Palsy" describing six cases he studied suffering from PD.(1)PD is one of the most common neurodegenerative diseases affecting approximately 1% of individuals over the age of 65 years and its incidence increases with age. Its symptoms are typically characterized by slow but progressive neurological and non-neurological disabilities such as tremor, memory problems, decreased sense of smell, stiffness, drooling, and constipation. Parkinson's disease is also commonly associated with other psychiatric illnesses, such as depression, anxiety, and psychosis. The neurological manifestations of the disease usually begin unilaterally; the most common is resting tremor which usually occurs in the hands before the feet. The hand would not move during walking, followed by the foot (on the same side) starting to scrub the floor. Subsequently the posture becomes more flexed and the steps shorter. The most common type of non-neurological manifestations is decreased sense of smell, an early symptom usually not recognized by patients. Other common signs include rapid eye movement (REM) disorder during sleep. Patients while sleeping may also have abnormal involuntary movements such as kicking, hitting and even screaming. Other symptoms exist but are less specific to Parkinson's disease, such as constipation and lethargy. Basically there are 4 main symptoms, at least 2 of which must be detected in order to diagnose that a patient is probably suffering from Parkinson's disease: (1)1 – Tremor at rest: Typically starts unilaterally in the upper limbs as tremors in the thumb or fingers. It can then extend to the lower limbs or…… to the center of the card……tt.2 – Weakening of the sense of smell. l3 – Decreased memory and ability to remember.4 – Excessive drooling and drooling, particularly during sleep.5 – Fatigue and insomnia.6 – Depressed mood Medical history1 – 10-year history of type 2 diabetes mellitus2 – Hyperplasia benign prostatic disease for 7 years3 - An episode of gout 3 years ago.4 - Chronic constipation for more than 3 years.5 - 2 year history of depression.Social history1 - Non-smoker2 - 1 unit of alcohol every nightDrug history1 - Citalopram 10 mg /day (selective serotonin reuptake inhibitor agent) for your depression2 – Metformin 500 mg/3 per day (biguanide agent) for your diabetic mellitus disease3 – Tamsulosin 0.4 mg/day (alpha blocking agent) for your hyperplasia benign prostatic
